 How to Refer a Patient

  • 1. Submit referral via online form, fax, or phone
  • 2. Include relevant clinical notes and imaging when available
  • 3. Referrals are reviewed within 24–48 hours
  • 4. Our team contacts the patient directly for scheduling
  • 5. Consultation notes are sent back to the referring provider
