How to Refer a Patient
- 1. Submit referral via online form, fax, or phone
- 2. Include relevant clinical notes and imaging when available
- 3. Referrals are reviewed within 24–48 hours
- 4. Our team contacts the patient directly for scheduling
- 5. Consultation notes are sent back to the referring provider
